What's inside
Pet details up front
Name, species, breed, sex, date of birth, colour/markings, microchip number, and the owner's contact — the identity block that makes the card usable at a boarding kennel or a new clinic.
A 10-row vaccination table
Date given, vaccine brand, batch/lot number, next due date, and the vet's signature on every row — the five columns that make an entry verifiable rather than a scribble.
Deworming and parasite prevention on page 2
A separate table for dewormers and ecto/endo-parasite preventives, so the products owners forget most keep a visible schedule too.
Why batch numbers matter — printed on the card
A short note explaining that the batch/lot number ties the dose in the animal to a specific vial — the detail that matters in a recall, an adverse reaction, or a dispute.
What this template can't do
A card in the owner's drawer reminds nobody. It records the next-due date; it doesn't message the owner when that date arrives, and it can't tell you which of last month's patients never came back. It also lives with the owner — the clinic keeps no copy unless your own records capture the same entry.
And note the quiet leak the card exposes: the batch number written on it is the same batch that should appear on that visit's invoice. If it doesn't, a vaccine was given and never billed.

The card goes home. The reminder — and the invoice — are your job.
CliniCore tracks every vaccination with its batch, WhatsApps the owner before the next-due date automatically, and its revenue-capture scan flags vaccines that were recorded but never reached an invoice — one of the most common leaks in small-animal practice.
